The Five Main Procedures of Panchakarma

In the Ayurvedic tradition, Panchakarma includes several purification procedures selected according to the individual’s constitution (Prakriti), current imbalances (Vikriti), digestive capacity (Agni), age, vitality, season and overall individual situation.
Not every procedure is appropriate for every person. The choice of treatment requires a prior assessment, and certain procedures may be contraindicated depending on health status, current medication or specific medical conditions.

Therapeutic Emesis

(Vamana )
Vamana is a classical Panchakarma procedure involving controlled and professionally supervised therapeutic emesis. In the Ayurvedic tradition, it is primarily associated with the management of Kapha-dominant imbalances, particularly when accompanied by heaviness, excessive mucus or congestion.
Classical Ayurvedic texts and traditional practice describe its use in selected respiratory, ENT, metabolic or skin-related patterns associated with Kapha. Its suitability, however, depends on several factors, including constitution, strength, digestion, age, season and general health.
Vamana is not routinely performed as part of every Panchakarma program. It is reserved for carefully selected situations following an individual assessment and requires specific preparation, supervision and post-procedure care.

Therapeutic Purgation

(Virechana)
Virechana is a classical controlled purgation procedure used within Panchakarma. According to Ayurveda, it is primarily associated with Pitta-related imbalances and traditionally forms part of the Ayurvedic process of digestive purification.
In traditional Ayurvedic practice, Virechana may be considered in selected patterns characterized by heat, acidity or certain skin and digestive manifestations associated with an imbalance of Pitta.
The procedure is preceded by individualized preparation and followed by a gradual dietary recovery phase known as Saṃsarjana Krama. The substances used, dosage and intensity of the procedure are adapted to the individual.
Virechana is not suitable for everyone and is performed only when a prior assessment indicates that the procedure is appropriate.

Therapeutic Basti

(Basti)
Basti occupies a central place within Panchakarma. It involves the rectal administration of specific Ayurvedic preparations, which may include medicated oils, herbal decoctions or other traditional formulations selected according to individual requirements.
In Ayurvedic understanding, the colon is closely associated with Vata Dosha. Basti is therefore traditionally used in the management of various Vata-related imbalances that may present with dryness, irregular bowel function, abdominal discomfort, stiffness or certain functional musculoskeletal complaints.
There are several forms of Basti, with different preparations, quantities, indications and therapeutic sequences depending on the intended purpose. A program may involve a single treatment or a series of sessions integrated into a broader Panchakarma protocol.
The protocol is determined following an Ayurvedic assessment and adapted according to constitution, digestion, vitality and the individual situation.

Ayurvedic Nasal Therapy

(Nasya)
Nasya is a traditional Ayurvedic procedure involving the administration of a small quantity of selected Ayurvedic oil or preparation through the nasal passages.
Within the Ayurvedic tradition, Nasya is primarily associated with the region above the clavicles — including the nose, sinuses, head and neck. It may be incorporated into an Ayurvedic program when the assessment identifies certain imbalances involving the nasal and ENT region, including sensations of congestion or heaviness in the head.
Depending on the type of Nasya selected, the traditional objective may be cleansing, balancing or nourishing. The preparation used and frequency of treatment are individualized.
Nasya does not replace appropriate medical assessment for persistent sinus problems, unusual headaches, neurological symptoms or any other condition requiring medical diagnosis.

Raktamokshana

(Traditional Bloodletting Therapy)
Raktamokshana is described in classical Ayurvedic literature as a procedure used for selected imbalances involving Rakta (blood) and certain patterns associated with Pitta.
Different methods are traditionally described. Among these, Jalaukavacharana — the application of medicinal leeches — is a specific technique used in carefully selected situations.
Within the Ayurvedic tradition, Raktamokshana is described in relation to certain skin, inflammatory and circulatory presentations. Its suitability must nevertheless be assessed individually, taking into account general health, medical history, medication, bleeding risks and possible contraindications. When medicinal leeches are used, appropriate standards of hygiene, safety and clinical monitoring are required.

The Complete Process

1. Preparatory Phase (Pūrva Karma)

The Panchakarma journey begins with Pūrva Karma, a preparatory phase designed to ready the body for deep detoxification by stimulating digestive fire (agni) and mobilizing stored toxins. This phase includes Deepana–Pachana to enhance digestion and burn accumulated metabolic waste, followed by Snehana (internal and external oleation) using medicated ghee and therapeutic oils to loosen toxins from tissues. Finally, Svedana (herbal steam therapy) gently dilates the channels, liquefying toxins and preparing the system for effective elimination. Pūrva Karma ensures that the body is optimally prepared for the main Panchakarma therapies, enhancing their safety, comfort, and efficiency.

2. Main Panchakarma procedure (Pradhāna Karma)

Pradhāna Karma is the core phase of Panchakarma, where one or more of the five classical detox therapies—Vamana, Virechana, Basti, Nasya, and Raktamokshana—are selected based on the individual’s dosha imbalance, age, strength, and clinical needs. These therapies remove deep-seated toxins, restore systemic balance, and address chronic conditions at their root cause.
